Catholic Charities provides a range of services to help Long Island seniors enjoy a high quality of life. We aim to ensure that seniors can live independently in comfort and safety while maintaining physical and emotional health. Social activities help seniors remain connected with the community, as well.

Services Provided

Senior Case Management
Senior case management and home care is also known as EISEP (Enhanced In-home Services for the Elderly Program). This program connects Nassau County seniors with services through government agencies, parishes, and community-based organizations. A case manager works with each senior to assess needs and develop and implement a service plan, which might include home care, transportation, state and local entitlements, and other needed services.

Meals-On-Wheels
Every weekday, the Meals-On-Wheels program delivers hot, nutritious meals to the homebound in Nassau and Suffolk counties. In Nassau, seniors may also participate in the frozen meal program, which provides meals seven days per week. Meals-on-Wheels case managers can assist the seniors in accessing other services they might need.

Senior Community Service Centers
Four senior community service centers in Nassau County offer transportation, hot meals, recreation, entertainment, education, health services, and socialization.

Congregate Meal Sites
Four congregate meal sites in Nassau and Suffolk counties provide hot meals, socialization, and recreation for seniors and the visionand hearing-impaired one to two days a week.

Caregiver Support Groups
In Nassau County our caregiver support group offers information, support, and an opportunity to share experiences to those caring for elderly relatives on an ongoing basis.

Bi-County Alliance of Senior Clubs
Seniors who want to become involved in their communities can contact the Bi-County Alliance of Senior Clubs. Sixty-seven senior clubs meet regularly in Nassau and Suffolk counties.

Catholic Charities' Senior Community Service Centers, Meals-On- Wheels, and EISEP are funded by Nassau County Department of Senior Citizen Affairs, Suffolk County Office for the Aging, NY State Office for the Aging, the U.S. Administration on Aging, and Catholic Charities, Diocese of Rockville Centre.

Eligibility and Fees

Seniors must be 60 years of age or older to participate in any senior services programs. Meals-On-Wheels are available to seniors who are homebound and unable to prepare meals.
A cost share may apply to EISEP services which is determined by a case manager during the assessment process.
There are small suggested donations for Meals-On-Wheels and the senior centers.
No one, however, is refused these services because of an inability to pay.
