BILINGUAL CASE MANAGER

We are seeking a talented, experienced bilingual (Spanish/English) case manager to join our mission and offer “Care with Dignity…Life with Hope” Based out of our Merrick office, in this full time position, the Case Manager will provide assessment, evaluation and referrals to supportive services for pregnant and/or parenting clients participating in Regina Maternity Service programs. The work schedule for this position is as follows: Noon – 8 PM (Monday-Friday). A benefits package accompanies the position which is grant funded.

Responsibilities include:

 Conduct telephone intake assessments, complete required documentation and follow up with clients referring them to appropriate resources.
 Complete home visits to assess needs, provide recommendations and prepare care plans.
 Assist clients with required paperwork for entitlement programs.
 Generate referrals to external resources.
 Interact with family and/or significant others regarding care.
 Monitor appropriate client care through follow up with agencies and families.
 Prepare accurate and timely progress notes.
 Transport clients to appointments when necessary.

Requirements include:

 Bachelor’s Degree in Human Services or related field OR 5 years’ of experience.
 Previous case management experience needed; 2 plus years preferred.
 Bi-lingual capability – Spanish/English – verbal + written.
 Ability to establish professional, productive relationships with other agencies.
 Ability to work a flexible schedule for client/family visits on occasion.
 Computer literacy including Microsoft Word and Outlook.
 A valid NYS Driver’s License.
